Skilled therapy services are a frequent source of concern about improper payments in the Medicare system. The concern about skilled therapy services can be extended to skilled nursing facility (SNF), outpatient therapy, and inpatient rehabilitation facility (IRF) services. Very often the issue is not whether the service itself was reasonable and necessary, but that the documentation provided does not support the skilled therapy as reasonable and necessary.  The lack of documentation often leads to the reviewer attempting to “read between the lines” when determining medical necessity.
